Sapling is building an AI operating system for independent medical practices. We connect patient communication, clinical documentation, medical coding, insurance, and billing into an autonomous healthcare team that takes work off clinicians’ plates.

We’re well-funded and building out our founding engineering team. This is an opportunity to work directly with the founders, own meaningful parts of the product, and help define how we build—from our AI architecture to our engineering culture.
What you’ll do
- Build and ship AI agents that handle real healthcare workflows, from practice operations and payments to care coordination.
- Own projects end to end: understand the problem, design the system, ship it, and improve it based on customer feedback.
- Build the infrastructure that makes agents dependable, including integrations, tool use, evaluations, monitoring, and human review.
- Work directly with practices to turn complex, often manual processes into software that people trust.
- Shape our architecture, technical standards, and engineering team as we grow.
What we’re looking for
- Strong software engineering fundamentals and experience shipping production systems.
- The ability to take an ambiguous problem and turn it into a practical, reliable product.
- Experience building with LLMs, agents, or applied ML—or a strong engineering background and evidence that you can learn quickly.
- Sound judgment about reliability, security, privacy, and when a workflow needs human oversight.
- A willingness to work across the stack and stay close to users.
- Clear communication, high standards, and a collaborative approach to solving problems.
Experience in healthcare, fintech, payments, developer infrastructure, or an early-stage startup is especially relevant. You don’t need to have worked in healthcare before.

Compensation
The annual base salary range for this role is 150,000–250,000, plus founding-engineer-level equity. Compensation within this range depends on experience, role scope, and responsibilities.
Location
We’ll work together from a Bay Area office on a hybrid schedule.
